Instructions:
  • Form may only be used for class adds and scratches of showing horses.
  • Use a separate form for each horse/rider combination.
  • You may make changes to multiple show weeks if the changes are the same for all weeks. Otherwise, submit a new form for each week.
  • If the rider is a “NEW” rider and is not listed on the entry blank, this form may not be used until the rider has been ‘officially' entered on
    the entry blank in the Show Office.
  • If the horse/pony is entering classes that require a measurement card, be sure that the measurement card has been presented to the
    Show Secretary.
  • If the rider is entering classes restricted to Amateurs, be sure that proof of USEF Amateur Status has been presented to the Show Secretary.
  • If the horse is entering jumper classes offering prize money, be sure that the horse has either an “Annual” or “Life” recording with USEF. A horse ID is not valid.
In order to be valid:
  • Form must be submitted prior to 5 pm for the class(es) the next day.
  • Form may not be used to add/scratch classes the same day the form is submitted.
  • Exhibitor is responsible for knowledge of eligibility requirements.
  • ‘Closed' classes may not be added/scratched with this form.
  • Rules and regulations concerning add/scratch and special classes will apply.
